Patient Information Card

The following Information card was designed to help Cystic Fibrosis patients navigate their state of health after starting a novel medication named Trikafta.

The general content was provided by the Subject Matter Experts in the hospital: Physical therapists, Nutritionist, Psychologist, Medical Doctor, Literacy specialist.

This Information Card is used at Children’s Hospital Colorado. It was also included in a Quality Improvement measure and presented to the Cystic Fibrosis Foundation.

Process of Development:

The lead coordinator for the project assembled the necessary content.
I drafted 3 formats for the card and presented them for the SMEs to choose. Once a format was selected, and using the organization specific logos and colors, I set out to organize the content.
During this phase, I once again presented several options of content presentation within the card. During our bi-weekly meetings, we would narrow down the content and discuss the language and visual presentation.
In the last iteration we consulted with a literacy and comprehension specialist to ensure that the text was reader friendly and unnecessary jargon was omitted.
Finally I adjusted font, contrast and size according to current visual accessibility guidelines.
